Application for the use of a guide dog in the facilities of Uppsala University

If you need to bring a guide dog or assistance dog into the university’s premises, you must apply for permission in advance. There are several basic requirements that must be met for this to be approved. In this form, you submit certificates confirming that you need a guide dog or assistance dog, as well as documentation showing that your guide dog or assistance dog has been trained and certified.

You must also confirm that the dog wears a service harness or service vest. If the basic requirements are deemed met, your case will be forwarded to the responsible manager, who will conduct a risk assessment of the relevant premises and decide on access.
